I have to re-enter my username and password every single time this message pops up (at least 3 times back to back) to send or receive emails. If I hit 'no' to the pop up box or cancel to the next username and password pop up, I am unable to receive or send mail until I close and reopen outlook.

Is there anything defined? Do you access your account with an e-mail client (Outlook, Thunderbird, Pocomail, etc), phone, notifier, or any other add-on/tool/web-site/etc? Is anything enabled in Settings->POP/IMAP?

Check Settings -> Accounts and Import -> Change Account Settings -> Other Google account settings -> Authorizing applications & sites.
